About Us

The National Neighbourhood Health Implementation Programme (NNHIP) is a transformational initiative jointly led by the Department of Health and Social Care (DHSC) and NHS England with NHS Horizons delivering in partnership with PPL, NAPC and NHS Confederation. It aims to improve health outcomes by addressing complex needs at a local level, especially in deprived areas where healthy life expectancy is lowest.

We know health is shaped by more than medicine — stress, loneliness, money worries, and where people live all matter. By working differently together, we can reduce health inequalities, improve wellbeing, and make neighbourhoods better places to live.   


The National Neighbourhood Health Implementation Programme (NNHIP) aims to do this. In 2025 an initial 43 places across the country were selected to join Phase 1 of the programme. Each of these were allocated a programme lead, who are working with existing local services to set up new Neighbourhood Health Services.  
 
Each place will also form a Neighbourhood Health Team consisting of community nurses, hospital doctors, social care workers, pharmacists, dentists, optometrists, paramedics, social prescribers, local government organisations and the voluntary sector – giving millions of people easier access to the right care on their doorstep.  


However we see Neighbourhood Health more than a programme, we see it as a community. We're bringing together people across health, social care, and the voluntary sector, united by a shared purpose: helping more people live well, not just treating illness when it happens. Which is why we’re encouraging everyone, not just our first 43 places, to get involved! Your neighbourhood is welcome to learn from our Phase 1 cohort, but you are also encouraged to join our monthly Community of Practice sessions, our Linkedin community, share your case studies of good practice and access resources from the programme.
