Bristol (South Bristol)
South West
South Bristol is one of six localities in the Bristol, North Somerset and South Gloucestershire geography. South Bristol have been developing a neighbourhood health and wellbeing approach through participation in the National Neighbourhood Health Implementation Programme (NNHIP). South Bristol locality are focusing on testing three neighbourhood models targeting working-age adults with multiple long-term conditions, aiming to shift from reactive care toward a more proactive, preventative and person-centred model.
Reflecting the diverse population of South Bristol, three neighbourhood models are being tested in three geographic areas to take interventions to residents based on their needs. All models are proactively identifying residents with escalating needs and engaging them through personalised conversations centred on "what matters to you", alongside individual/ group interventions, better joining up of health and wellbeing services and addressing the wider determinants of health for this population. This work builds on community engagement and partnership working that has been a strength in South Bristol's way of working for many years.
Strong partnership working underpins the programme, with established governance through the South Bristol Locality Partnership Board and collaboration across Primary Care, Community services, Social Care and the Voluntary Sector. Alongside neighbourhood service design and delivery, South Bristol is investing in long term evaluation of resident outcomes. These efforts aim to generate learning from rapid test-and-learn cycles and develop a scalable neighbourhood model that improves coordination and outcomes for residents.
