Cornwall and the Isles of Scilly (CIOS) is one of the few systems nationally delivering the Neighbourhood Health and Wellbeing model at full scale through the National Neighbourhood Health Implementation Programme (NNHIP). Building on a strong history of partnership working and localism, CIOS has mobilised Integrated Neighbourhood Teams (INTs) across all 16 neighbourhoods, starting to bring together primary care, community services, mental health, local authorities and the voluntary sector around shared populations.  This has been done in 2 waves, the first wave of 5 neighbourhoods are all showing promising impact on outcomes. 

The programme is focused on strengthening relationships, developing local leadership, and enabling teams to respond to the needs and strengths of their communities. Alongside this, CIOS has developed a draft system-wide Blueprint for Neighbourhood Health and Wellbeing, setting out a clear long-term vision, principles and delivery approach. 

A key area of innovation has been the development of a robust impact and evaluation framework, including deep dive reviews that are now informing national learning. Despite significant operational pressures, the system has maintained momentum by balancing local, bottom-up innovation with a shared strategic direction.
