Coventry’s National Neighbourhood Implementation Programme is transforming how health and care services are delivered by developing a population based, neighbourhood model. A Population Health Management diagnostic focused on the 7% of Coventry residents who account for 76% of secondary care and adult social care activity was undertaken, highlighting a core cohort with complex needs. From this, seven key themes were established to guide the new model of care.
Through extensive partnership work, 39 neighbourhood geographies have been created and grouped into six Integrated Neighbourhood Teams (INTs) and one Coordination Hub with place level approval and sighting occurring at care collaborative and Health and Well Being Board. The first INT in South-West Coventry went live in December 2025, alongside an INT Care and Support Forum and the first iteration of the Coventry Coordination Hub; initially focused on UEC pathways.
Over 2026–2027, the programme will refine and scale INT delivery, strengthen UEC integration at place and develop the Coordination Hub. Key enablers include estates planning, digital infrastructure, workforce redesign and large-scale engagement to support expansion from one to six INTs by March 2027.
This model will provide hyper local, integrated support, enabling a stronger, more continuous relationship with residents. By building coordinated teams around neighbourhoods, Coventry is creating a sustainable approach to meet the needs of an ageing, multi morbid population and improve outcomes.
