West Leicestershire, as part of the wider Leicester, Leicestershire and Rutland system (LLR), is focusing on developing and delivering neighbourhood-led transformation as a stimulus for building integrated workforces and closing care pathway gaps, improving community resilience, reducing pressure on acute services and supporting patients to play a key role in their own care planning. 

Our approach demonstrates commitment to tackling health inequalities, particularly for those with one or more Long-term conditions, supported by risk stratification and proactive outreach through Local Authority, Primary and Community Care and VCSE partnerships. 

Mature Integrated Neighbourhood Teams (INT) are already in place with robust governance, established collaboratives, and devolved decision making. 

Focused data capture and reporting supported by strong Data Sharing Agreements is in place and has enabled the development of MDT’s, led jointly by GP Federations and the Local Authority Care Coordinators to support improved patient care planning and patient involvement and activation. 

Impact is being evidenced through greater collaboration, overcoming barriers and challenges through the development of improved processes and care pathways. 

This initial phase will be scaled and spread across the other neighbourhoods, sharing learning and good practice as key components of the blueprint for neighbourhood care services and integrated working across LLR.
