Morecambe Bay is a large, dispersed area with a population of c. 370,000 across urban, coastal and rural communities and a combination of highly deprived and more affluent areas. There is significant variation in life expectancy and years spent in good health (across both gender and geography). There are nine neighbourhoods, each matched to the footprint of a Primary Care Network (PCN) with a supporting Integrated Care Community (ICC).  

Our approach to neighbourhood health is rooted in: 

  • Risk stratification to identify cohorts and tailor support offers 

  • Promoting independence and avoiding unnecessary hospital admissions / long-term residential care 

  • Listening to resident voices and responding to what matters most to them 

Examples of neighbourhood health include: 

  • Integrated Wellness Service: coordinated, holistic, intensive and early interventions from a multidisciplinary team for residents with 3+ emergency admissions in 12-months or long lengths of stay (14+ days). 

  • Morecambe Bay Respiratory Network: a multidisciplinary team across primary, community and secondary care providing integrated care for patients with suspected/confirmed chronic respiratory disease. It focuses on earlier intervention and support to keep residents living well.  

  • Integrated Care Communities: teams of health and care workers, voluntary organisations and wider community groups who practice population health with a mobilised community with a combination of routine activities and localised actions that reflect the needs of each community.  

  • Resident-led initiatives: supporting communities to address wider determinants of health and engage with health and care services
