North East and Yorkshire

In North East Lincolnshire (NEL), we’re working with local partners, communities and voluntary organisations to improve outcomes for people with multiple longterm conditions and those at rising risk. 

We’ve introduced new neighbourhood huddles that bring teams together to understand people’s needs, coordinate support, and focus on what matters most to them. Using local insights and data, we are identifying those who would benefit most from joinedup care and supporting them earlier. 

Alongside this, we are laying the groundwork for future Neighbourhood Health Centres by talking to communities and our workforce. We are also working with the NHSE digital team to improve operational interoperability exploring what we need to change to ensure our digital systems help support integrated working.  

Although national guidance is still developing, the programme is already strengthening relationships. Over the coming months we are working towards how we scale integrated neighbourhood working across NEL.
