Shropshire’s neighbourhood health approach builds on an already strong foundation of place‑based working, strengthened further through learning from the national programme. The partnership brings together NHS Shropshire, Telford and Wrekin, Shropshire Council, primary care, community and mental health services, and the voluntary and community sector, working collectively to improve outcomes through insight‑led neighbourhood action. 

Shropshire’s rural geography, ageing population and workforce pressures continue to shape both the challenges and opportunities for neighbourhood health, particularly around access, prevention and supporting people to stay well at home. Since joining the programme in September 2025, have worked together to develop clear, data‑driven priorities for their integrated neighbourhood teams. The North Shropshire neighbourhood are focusing on diabetes, Shrewsbury and the South-East Shropshire are prioritising frailty, and the South-West and Rural areas are concentrating on isolation and long‑term conditions management. 

Early activity is already showing impact. In Shrewsbury, the frailty team has begun meeting patients for introductory assessments to build relationships and identify needs, including mobility tests and lifestyle discussions. From the first week the team had already supported patients to access social prescribing, audiology and falls prevention services. This early intervention reflects neighbourhood working at its best, ensuring support is shaped around what local people need.
