Following successful confirmation as a Wave 1 NNHIP site in September 2025, the local partners in Stockton-on-Tees—including the GP Federation, Integrated Care Board, Local Authority, Public Health, NHS Foundation Trust partners, VCSE organisations, and academic and regional stakeholders— moved quickly into mobilisation. Leadership roles were appointed, governance structures and a cross-system Steering Group were established, and core programme infrastructure and enabling workstreams were created, focusing on data and evaluation, model design, community voice, and communications.
Through late 2025 and early 2026, the programme focused on engagement, data development, and testing new neighbourhood-based ways of working. Professional and VCSE engagement sessions were held, community voice activities began, and population health management tools were used to identify priority resident cohorts. In early 2026, the first Integrated Neighbourhood Team (INT) delivery started, using a “test, learn, adapt” approach informed by resident and professional feedback. Evaluation processes and data-sharing arrangements were introduced, and the programme continued refining governance and delivery as learning emerged. In March 2026, Stockton was confirmed as an incubator site for neighbourhood population health management tool development, while ongoing INT delivery, learning events, and national engagement continue to shape the programme’s development and future impact.
