Sunderland is transforming its health landscape by shifting to a proactive, neighbourhood-led ecosystem. Our approach is built on a legacy of deep-rooted collaboration, bringing together the NHS, local government, the VCFSE and local communities as equal partners. 

Driven by the need to address significant health inequalities and a high proportion of older residents, we are focusing on integrated frailty management to deliver care closer to home. Within this framework we are: 

  • Uniting neighbourhood care to deliver our mission to ensure everyone in Sunderland lives a healthy, happy life, ensuring no one is left behind. 

  • Scaling core neighbourhood health components, including Integrated Neighbourhood Teams (INTs), modern general practice, and urgent community services such as the Acute Frailty Service, which has already demonstrated high levels of admission avoidance. 

  • Using advanced data-sharing to identify high-risk cohorts and building on internationally recognised digital infrastructure to empower residents and promote independence. 

  • Accelerating our ambitions to put prevention at the forefront of neighbourhood delivery by building capacity and community connection through local assets, strengths-based delivery and whole system engagement.
