The System Archetype document Fit for the Future: Towards Population Health Delivery Models has been published. This publication sits within the ambition of the 10 Year Health Plan to redesign the NHS around prevention, community-based support, and a more digitally enabled, proactive model of care.

In summary, the document sets out:

  • Sets out a vision for future population health delivery, aligning with the plan’s three major shifts: hospital to community, analogue to digital, and sickness to prevention.

  • Describes emerging system archetypes that will help local systems organise care around population needs rather than organisational structures.

  • Draws on extensive engagement from staff, public, health leaders and system partners, ensuring the direction reflects real operational experience and population insight.

The system archetypes are designed to help leaders think differently about how care can be delivered, using more integrated, outcomes focused, and digitally enabled approaches.
